Running Prism for OS X, nothing appears on the toolbars. I see only three blank rows.

When you have two monitors mounted side by side, Prism 3cx works fine if the left monitor is the primary one. But if the right monitor is primary, you'll see an empty toolbar. We have fixed this in Prism 4.
